When I called for an itemized bill after having my second, I was told that it doesn't matter what's on there, the contract calls for a specified amount for a c-section, and that's the amount that was billed and paid, and that's the amount that my co-payment is based on. So the details didn't matter at all. Although it still irks me that a couple of (minor) things on the bill are wrong! It’s fascinating how different the various insurance plans are.
