The form is a catalina print out NOT display. You can decide to return the items or try again art a different time when the rude lady is not there. Good luck! Diana,

I called Kmart after i got home (thought i could print out 1 at home) and they said i could come in and they would print out a form. Went back today and the very rude woman didn't know what i was talking about. I even took the smart source page with me. She insisten the " candy lady" said there was a form on the display.
